1st order page incorporated - I need the code

I lost my file containing the code for 1st order page incorporated that I used before
in Fantasos.

Now when I purchased the mindmap for 'free report - OTO - ebook' setup,
I need the code to put the form from 1st order page into my squeeze page.

Can you please give me the complete code for 1st order page incorporated?

You get this code in Packages. It's one of buttons for the package.

Noone can give you the code, since it's package dependent.

Go to Store->Packages.

Find the Package you want.

Then click on the 'inc" link.

An alert box will open.

This is the code you need and as Andre said, it is Package specific.
